It is Observed that many cases Of BSF officers submitted to MHA for [N 2) Dumb
consideration to various UN Mission on secondment basis are being 7 received
back from MHA [PMA Cell) due to non completion of P-II Form/ EAC Form /
Supplementary Sheet etc.
In this connection, the following documents received from MHA (Policemll
Division) PMAaTFm forwarded herewith With the request that while applying for
various UN Mission on secondment basis, the following documents be forwarded
alongwith nominations duly filled in all respect (ensuring that not column be
left) and pasting a passport size photograph on front page of PH Form, so that
same can be submitted to MHA, for consideration and to avoid any objection at
later stage:
Form 011 prescribed proforma
UN EAC Form (Employment and Academic Ccertification)
Employment fecord— Supplementary Sheet
Certificate (to be completed by the Relevant local authority)
Bio Data of concerned officer
Further, it is to inform that incomplete nominations of offcers not repeat will
not be entertained.
